Indication
Esoral 40 is a medication that lessens the quantity of acid your stomach produces. Heartburn, acid reflux, and issues with your food pipe are all treated with it. Stomach ulcers can also be prevented and treated with them. Esoral 40 should be taken as prescribed by your doctor.
Pharmacology
The H+/K+ ATPase in the gastric parietal cell is inhibited by the proton pump inhibitor esomeprazole, which reduces the amount of stomach acid produced. Esomeprazole (omeprazole's S-isomer), the first proton pump inhibitor with a single optical isomer, offers better acid control than racemic proton pump inhibitors.
Dosage & Administrations of Esoral 40
Erosive esophagitis-
Maintenance of healing of erosive esophagitis-
Risk reduction in NSAID-associated gastric ulcer-
H. pylori eradication (Esomeprazole MUPS tablet with 1000 mg Amoxicillin and 500 mg Clarithromycin)-
Zollinger-Ellison syndrome and idiopathic hypersecretion-
Children 1-11 years:
Erosive esophagitis: Weight <20 kg: 10 mg once daily for 8 weeks. Weight ≥20 kg: 10 mg or 20 mg once daily for 8 weeks
Maintenance of healing of erosive esophagitis: 10 mg once daily
Children below the age of 1 year: Esomeprazole Mups tablet is not approved for use in children younger than 1 year.
Esomeprazole tablet or capsule: should be swallowed whole and taken one hour before a meal.
Delayed-Release Oral Suspension uses guidelines: The entire packet's contents should be placed in a little glass with 15 ml of water. After thoroughly stirring the mixer, give it two to three minutes to thicken. Within 30 minutes, stir once more and consume. Add extra water, swirl, and drink right away if any medicine is still present after consumption. If the suspension is to be taken via a nasogastric or gastric tube, 15 ml of water should be placed in the syringe. After shaking the syringe, wait two to three minutes for the suspension to thicken. Within 30 minutes, shake the syringe and inject it into the stomach using a nasogastric or gastric tube. Make sure to use a syringe that is the right size. Shake and ingest any leftover substances through the gastric or nasogastric tube.
Esomeprazole IV Injection: Esomeprazole IV needs to be injected slowly into the vein. The solution for IV injection is made by adding 5 ml of the supplied solvent (WFI) to the vial. The injection should be administered carefully over a minimum of three minutes following reconstitution. If the solution is kept at room temperature up to 30°C, it needs to be used within 12 hours of reconstitution. There is no need for refrigeration. If the reconstituted solution has visible particles, don't use it.
Esoral 40 usage over an extended period of time may result in weak bones and a mineral deficit, especially magnesium. Consume enough calcium and magnesium in your diet or take supplements as directed by your doctor.
If you experience decreased urine, edema (swelling brought on by fluid retention), lower back discomfort, nausea, exhaustion, rash, or fever, call your doctor immediately once. These might be kidney-related symptoms.
Interaction of Esoral 40
CYP2C19 and CYP3A4 substantially metabolize esomeprazole in the liver. Esomeprazole does not appear to inhibit CYPs 1A2, 2A6, 2C9, 2D6, 2E1, or 3A4 in vitro or in vivo investigations. There should be no clinically significant interactions with medicines processed by these CYP enzymes. Esomeprazole has no clinically significant interactions with phenytoin, warfarin, quinidine, clarithromycin, or amoxicillin, according to drug interaction studies.
Gastric acid secretion is inhibited by esomeprazole. As a result, Esomeprazole may interfere with the absorption of medications whose bioavailability is influenced by gastrointestinal pH. (e.g., ketoconazole, iron salts and digoxin).
Oral contraceptives, diazepam, phenytoin, or quinidine appear to have no effect on Esomeprazole's pharmacokinetic profile.
Clarithromycin in Combination Therapy: When esomeprazole, clarithromycin, and amoxicillin are given together, the plasma levels of esomeprazole and 14-hydroxyclarithromycin rise.
Contraindications
Esomeprazole is contraindicated in patients with known hypersensitivity to any of the formulations.
Side Effects of Esoral 40
When compared to short-term treatment, there is no difference in the categories of linked adverse events noticed with maintenance treatment up to 12 months.
Pregnancy & Lactation
The manufacturer advises caution. It is not known if Esomeprazole or its metabolites appear in human breast milk.
Precautions & Warnings
If the patient has severe liver failure and renal impairment, esomeprazole should be administered with caution. Proton pump inhibitors, such as Esomeprazole, may marginally increase the risk of hip, wrist, and spine fractures, especially when used for more than a year.
